Complete Forms

1. Download the Patient Consent Form and complete it with your patient.
2. Download and complete the applicable order form(s) for the services you require.

Submit Forms

Fax the completed Patient Consent Form and all order forms to (416) 962-MFAC (6322).

If you are a first-time client, a MAXFACS representative will contact you to set up a file transfer account for the transfer method of your choice.

Upload Files

Upload your DICOM files to the MAXFACS server.

The time needed to complete this transfer will vary depending on the number and size of your files, your internet connection speed, and the method of transfer.

Confirmation

A MAXFACS representative will contact you via e-mail to confirm that your DICOM files have been received.

You will receive another e-mail within 24 hours to notify you that your case has been processed and is ready for download.
